I confirm and understand that Plastic Surgery Thailand will only proceed with my booking upon receipt of  both (i)Registration Fee of A$395.00 (ii)  Booking Form and  Disclaimer.

I also  understand that my Plastic Surgery Thailand  registration fee of $395 is for services provided and is not refundable. Travel and Accomadation is only confirmed upon payment of all applicable costs. In order to confirm my Travel / accommodation,  I will pay  for these services 30 days prior to departure.
I understand that I pay the cost of my Procedure and the Hospital Account after the pre surgery check.
